The successful candidate will support to our adult support services, working closely with community partners, GP practices and other healthcare professionals. The Social Prescriber/support worker will provide a pathway for clients from referral to Community and Voluntary Sector supports.

The successful candidate will provide efficient and organised administrative support to our, Hospital Discharge Support, Community Transport, and Core Management teams.

Applicants should have good communication skills, relevant administrative experience, familiarity, and relevant skills with Microsoft Office products as well as experience of recording and reporting of information systems. The role would suit someone who is well-organised, motivated, and digitally confident, as well as being a people person who enjoys being part of a team and would enjoy being involved in many areas of the organisation.

Location: Employment will mainly be based in the hospital acute sites, community hospital and rehabilitation units across Lanarkshire.

For this post we are seeking a highly motivated individual who would engage with patient, families, and carers to offer advice on possible community supports and refer to appropriate organisations when required. The service aim is to encourage clients to engage with additional supports with the anticipated outcomes of reducing loneliness and isolation, improving self-management of conditions, encouraging independence and physical activity. The service offers support throughout the patient’s journey, and this can continue post discharge. This service is hospital based.
